END USER DECLARATION FORM FOR ULVAC CUSTOMERS & END USERS
End User Declaration (EUD) Form
Dear Valued Customer,
As part of ULVAC Technologies, Inc.'s commitment to maintaining the highest standards of compliance with U.S. laws and federal regulations, we require all customers to complete an End User Declaration (EUD) form. This policy ensures that ULVAC adheres to the rules governing the transfer, export, sale, and disposal of any products or technical data originating from the United States.
In particular, ULVAC is committed to avoiding transactions with embargoed countries, such as the Crimea Region of Ukraine, Cuba, Iran, North Korea, Syria, and Venezuela, as well as ensuring that we do not conduct business with denied entities. Additional scrutiny is applied to parties in Russia, especially in sectors like oil and gas, which may be subject to specific restrictions. Sanctions on individuals in Russia also remain strictly enforced.
Further, transactions involving China and Hong Kong may require export licenses based on the nature of the end users or end uses, especially for advanced computing or semiconductor manufacturing items. Additionally, ULVAC strictly prohibits the sale of products for the development or proliferation of weapons of mass destruction, including missile technology, nuclear weapons, biological or chemical weapons, and other controlled applications such as UAV (Unmanned Aerial Vehicles).
To ensure full compliance with these regulations, we ask that all ULVAC customers submit the appropriate EUD form based on their country of origin. This form helps us verify that our products are used for lawful and approved purposes, in accordance with U.S. export controls and sanctions. EUD forms are required to be refreshed every six months.
